OCHA requires medical reports for students who will be attending on-campus classes or school events. The attached medical form must be completed by you and signed by your child's doctor and returned to the school office or submitted here once complete.

In compliance with the State of Missouri and the Clay County Health Department OCHA requires records of each student's immunizations
(or immunizations exemption form).
